Indian Stream Health Center wishes to honor a service organization which: is nonprofit and community based, addresses the needs of area residents, and has made a long term commitment to social improvement.

Instead of honoring an Organization, the Community Health Advocate Award was created to honor an Individual who serves the needs of the community.
 
ISHC wishes to honor an individual who exemplifies service and has committed his/her time and talents to identifying and addressing community health needs.
