OUR LEADERSHIP TEAM
With a broad and extensive range of experience and skills, the leadership team at Indian Stream Health Center (ISHC) provides strategy and day-to-day operational oversight of the area’s most comprehensive community health center.
Jonathan W. Brown, MBA
Chief Executive Officer (CEO)
Mr. Brown serves as Indian Stream Health Center’s Chief Executive Officer, a role he has held since 2015. At ISHC, he oversees daily operations and directs strategy and growth for the practice. He also serves as a significant public “face” of the organization by representing ISHC in regional healthcare projects, with board volunteers, elected officials, business leaders and other stakeholders.
Prior to being named as CEO, Mr. Brown held several leadership roles at ISHC including Finance Director and Information Systems Manager. In those positions, he led numerous organizational initiatives such as an electronic health record implementation, implementation of an updated information technology infrastructure, a capital campaign, three capital projects (including major renovations, a significant building addition and a new access point (our office in Canaan, VT.), patient-centered medical home accreditation, and a revenue cycle department transformation. In all, he has over ten years of experience in New Hampshire’s health care environment.
As an active community volunteer, Mr. Brown serves on number of boards and committees including: board volunteer for the North Country Chamber of Commerce (including one-year as president), the North Country Health Consortium Board and the North Country Accountable Care Organization Board. He is a graduate of the Bi-State Primary Care Association Leadership Development Program, the George Washington University Geiger Gibson Leadership Development Institute, North Country North of the Notches Leadership Program, Neil and Louise Tillotson Grantee Learning Community, and the Office of Rural Health Policy Rural Voices Leadership Program.
He holds a Master of Business Administration Degree and a Bachelor of Science Degree in Information Technology/Business Systems Analysis from the University of Phoenix in Phoenix, AZ. Mr. Brown is a second generation FQHC employee. His mother currently works for Family Health Center of Worcester, MA located in the neighborhood where he was born and raised. He has been a life-long patient of community health centers and through his experience as an FQHC executive, has been empowered to continue to advocate and support for equitable and high-quality health care for everyone.
Chief Financial/Operations Officer (CFO/COO)
Mr. Rae serves as Indian Stream Health Center’s Chief Financial & Operations Officer, a role he has held since 2015.
Prior to joining ISHC, Mr. Rae was a strategic leadership consultant, helping companies and individual entrepreneurs with business planning, organization and financing. Working with over 100 organizations in the United States, Canada, Africa and Europe in C-Suite positions, he has an extensive range of leadership skills and experiences in the areas of finance, operations, design, production, marketing and sales.
Mr. Rae holds a Master of Science Degree in Business Education from New Hampshire College (now known as Southern NH University) in Manchester, NH and a Bachelor of Science Degree in Accounting from Merrimack College in Merrimack, MA. He was a Certified Public Accountant and is a graduate of the Kennebec Leadership Institute in Kennebec, ME. He is a graduate of the Sarasota School of Massage in Sarasota, FL.
Born in Cape Town, South Africa, Mr. Rae moved to the United States as a young adult. He seeks to use his variety of experiences to contribute to ISHC in many ways.
Sharon Cleveland, BSHA, SHRM-SCP
Director of Human Resources (HR) & Compliance
Ms. Cleveland serves as Indian Stream Health Center’s Director of Human Resources & Compliance, a role she has held since 2014. At ISHC, she is responsible for leading all aspects of Human Resources and Compliance implementation and training.
Prior to being named as Director of Human Resources & Compliance, Ms. Cleveland served as ISHC’s Administrative Supervisor and Compliance Officer.
In all she has been at ISHC for over a decade and has played an integral part in the Center’s transformation into a highly regarded employer and community resource. Before joining ISHC, Ms. Cleveland held leadership roles at Paul Cleveland Trucking in Colebrook, NH and at the North Country Chamber of Commerce in Colebrook, NH.
She holds a Bachelor’s Degree in Health Care Administration and an Associate’s Degree in General Studies from Granite State College in Manchester, NH. She is currently pursuing a Master’s of Business Administration Degree from Fayetteville State University in Fayetteville, NC. She is a Senior Certified Professional in the Society of Human Resource Management and is a graduate of Harvard University’s School of Medicine Executive Leadership Program.
Ms. Cleveland is passionate about the community, ISHC employees, and the population that ISHC serves and continues to remain dedicated to the overall vision and mission of the organization. She has served, and continues to remain active, on many organizational and community boards and committees.
Laurie Connors, LICSW
Behavioral Health Director
Ms. Connors serves as Indian Stream Health Center’s Behavioral Health Director, a role she has held since 2017. At ISHC, she leads the Behavioral Health Team in providing a wide range of integrated services to those it serves as part of an overall wellness approach to health care.
Prior to being named as ISHC’s Behavioral Health Director, Ms. Connors served as a Behavioral Health Counselor for the practice. She has been at ISHC since 2014. Before joining ISHC, Ms. Connors served as a Supportive Services Counselor at Hope Hospice in Fort Myers, FL. She has also held leadership roles at Turoc Family Caregiving in Melbourne Beach, FL, Volunteers for the Homebound and Family Caregivers, Inc. in Boca Raton, FL and at the Center for Emotional and Behavioral Health in Vero Beach, FL.
As a Licensed Independent Clinical Social Worker (LICSW), Ms. Connors is trained in Eye Movement Desensitization and Reprocessing (EMDR) for trauma work and Post Traumatic Stress Disorder (PTSD). She also utilizes Cognitive Behavioral Therapy (CBT) with mindfulness to treat anxiety, depression, stress, OCD and substance abuse issues.
She holds a Master’s Degree in Social Work and a Bachelor of Science Degree in Health Administration from Florida Atlantic University in Boca Raton, FL.
Ms. Connors believes that all healing begins with building relationships and is excited that ISHC is a leader in treating the whole person. She loves to travel, enjoys living and working in this community and loves it when it snows.
Mr. Crawford serves as Facilities Manager at Indian Stream Health Center, a role he has held since 2017.
At ISHC, he oversees plant management staff and maintenance for the practice’s facilities in Colebrook, NH and Canaan, VT.
Prior to joining ISHC, he served as Plant Manager for Coos County Nursing Hospital in West Stewartstown, NH. He has held other roles at The Balsalms Hotel in Dixville Notch, NH and North Country Plumbing and Heating in Colebrook, NH.
He holds an M.A. from Northeastern Technical College in Boston, MA and is certified in Refrigeration/Air conditioning, oil heat, gas, underground fuel storage, Motor vehicle, R-410A, Universal License. He is a Master Plumber.
As an active community volunteer, he has coached soccer, basketball and baseball and been involved with the Cub Scouts. His favorite thing about working in the health care field is being able to put a smile on someone’s face.
Peter Moran, MD
Dr. Moran serves as Medical Director at Indian Stream Health Center, a role he has held since 2017.
At ISHC, Dr. Moran oversees and manages the practice’s clinical staff in addition to taking care of his own panel of patients.
Dr. Moran has been at ISHC for over 10 years. Prior to assuming his current leadership role, he served as a family practice staff physician.
Before joining ISHC, he was a Family Physician in private practice in Haverhill, MA Other clinical posts have included Primary Care Physician at Hallmark Health and Family Medicine provider in the United States Air Force.
He holds an MD from the University of Massachusetts Medical School in Worcester, MA where he also completed his Residency. He earned a Bachelor of Science Degree in Chemistry from Northeastern University in Boston, MA.
Dr. Moran is currently pursuing a Master’s Degree in Health Care Administration from the Harvard T.H. Chan School of Public Health. He also serves on the board of directors for Upper Connecticut Valley Hospital
He plans to finish his career in the North Country re-inventing health care with ISHC.
Jordan Phinney, MHA
Patient and Financial Services Director
Ms. Phinney serves as Patient and Financial Services Director at Indian Stream Health Center, a role she has held since 2015. At ISHC, she oversees and manages the Patient Services Department, which includes registration, billing and medical records staff and policies and procedures. Ms. Phinney also provides management oversight of revenue cycle processes, training, credentialing, claims management, collections, insurance, integrity of patient accounts, accounts receivable management and utilization analysis, as well as risk management and patient experience opportunities.
Ms. Phinney has been at ISHC since 2013. Prior to assuming her current leadership role, she served as Outreach and Enrollment Facilitator, as well as Outreach Supervisor, for the practice. Before coming to ISHC, she served for eight years as a Medical Laboratory Scientist at Upper Connecticut Valley Hospital in Colebrook, NH.
Ms. Phinney holds a Master’s Degree in Healthcare Administration from Saint Joseph’s College in Standish, ME, a Bachelor’s Degree in Medical Laboratory Science from the University of Cincinnati in Cincinnati, OH and an Associate’s Degree in Applied Science in Medical Laboratory Technology from Clinton Community College in Plattsburgh, NY. She is a graduate of the Bi-State Primary Care Association Leadership Development Program.
As an active community volunteer, Ms. Phinney is involved in a variety of organizations including serving as Chair of the Advisory Board for the town of Colebrook Recreation Department.
She seeks to ensure that everyone receives quality health care regardless of any socioeconomic status or situation and to help those who cannot help themselves, making the world a better place through providing access to innovative health care.
Jessica Remillard, PharmD, R.Ph
Ms. Remillard serves as Pharmacy Director at Indian Stream Health Center, a role she has held since 2016. At ISHC, she is responsible for directing, supervising, coordinating, and controlling the operation of the pharmacy department.
Ms. Remillard has been at ISHC since 2015 and served as Staff Pharmacist prior to assuming her current leadership role. Before coming to joining ISHC, Ms. Remillard worked at Rite Aid Pharmacy for several years where she served as a staff pharmacist.
She holds a Doctorate of Pharmacy from the Massachusetts College of Pharmacy in Boston, MA
She and her team are pleased to embrace a daily mission to provide the highest quality of comprehensive medication services to ISHC’s patients and the community at large.
Information Systems Director
Ms. White serves as Information Systems Director at Indian Stream Health Center, a role she has held since 2012. At ISHC, she manages the practice’s Electronic Medical Record (EMR) as well as all Information Systems (hardware, software, networking, and communications) at all locations of ISHC. She works closely with the operational side of management to ensure systems are running and in place for all staff, and works with the clinical side of the organization to ensure all processes, data metrics, and tools are available and utilized to meet the needs of the organization and its stakeholders.
Ms. White has been at ISHC for seven years and served as a specialist in the Information Services Department before assuming her current leadership role. Prior to joining ISHC, she led Hazardous Design, a screen printing and design company in Colebrook.
Ms. White holds a Bachelor of Science Degree in Chemistry and Physics from Keene State College in Keene, NH.
Building, configuring, and improving systems are truly the elements that drive Ms. White each day as she seeks to continue to advance her knowledge and experience in the health care information systems space. She hopes to be at the forefront of health care system and data innovations as the nation’s health care system evolves in the years ahead.
