Peter Moran, MD - Medical Director
Dr. Peter Moran is the family practice physician at ISHC. He came to the health center four years ago after serving 7 years as a medical officer in the Air Force and briefly having a private practice in MA. Dr. Moran believes that a team approach to the delivery of primary care is very important; he feels he has found his professional niche at ISHC and his personal lifestyle meshes nicely in the North Country where he is happy to call home.
Elizabeth Sweeney, DO
Dr. Sweeney is an osteopathic physician who is dual board certified in Family Medicine and Neuromusculoskeletal / Osteopathic manipulative medicine with special interest in women's health including prenatal care, pediatrics, and preventive medicine. A native of Indiana, she comes to ISHC by way of Bangor, Maine. Dr. Sweeney and her partner Ben were happy to relocate to Northern NH where Ben can pursue his career in Forestry and she can fulfill her dream of working in a rural health care clinic with like-minded providers dedicated to providing high quality and compassionate care.
Jayne Tarkleson-Berube, DO
Jayne Tarkleson-Berube is a great addition to Indian Stream's care teams, providing pediatric services to our patients. Jayne is a native to Colebrook and is "excited to be back in the area in which [she] grew up". Jayne looks forward to reconnecting with people and perhaps caring for the children of childhood friends. Always working with a smile, she is scheduled to be at the health center every Thursday, and works at Weeks Medical Center for the remainder of her week.
Mary Judd, PA-C
Mary Judd is a very experienced physician assistant having served the community for 18 years. A member of Dr. Fothergill's team, Mary is deeply devoted to her patients and is a champion for the ISHC Women's Health Program which is being developed in collaboration with UCVH. Mary and her family are happily settled in Colebrook where they plan to stay for the long term.
Mackenzie Dufour, PA
Ms. Dufour holds a Master of Physician Assistant Studies from the Massachusetts College of Pharmacy and Health Sciences in Manchester, NH and a Bachelor of Arts Degree in Biology and Chemistry from St. Joseph’s College of Maine in Standish, ME. She holds a professional license in New Hampshire.
Linne Humbargar, PA
Ms. Humbargar comes to ISHC from North County Primary Care (NCPC) in Littleton, NH. She holds a Master of Medical Science degree from St. Francis University in Loretto, PA and a Bachelor of Science Degree in Business Administration and Marketing from Colorado State University in Fort Collins, CO. Ms. Humbargar has been recognized for Excellence in Medicine from Evans Army Hospital in Ft Carson, CO. She and holds professional licenses in New Hampshire, Colorado, and California.
Yoomi Lee, ARPN
Ms. Lee joins ISHC from Educerus Health in New York where she worked as a Nurse Practitioner. She holds a Master of Science Degree in Nursing from the University of Nebraska in Omaha, NE, a Bachelor of Science Degree in Nursing from the Massachusetts College of Pharmacy and Health Sciences in Boston, MA and a Bachelor of Arts Degree in History from Northeastern University in Boston, MA. She holds professional licenses in New Hampshire, Vermont, Massachusetts and New York.
Laurie Connors, LICSW
Laurie is glad to be back in a smaller community setting. Laurie approaches behavioral health from the bio-psycho-social-spiritual perspective that assists clients to utilize their own strengths in achieving wellness. Laurie is well-versed in treating many issues and specializes in depression, anxiety, loss & grief. She is trained in the latest use of EMDR (Eye Movement Desensitization & Reprocessing) therapy for Post-Traumatic Stress Disorders (PTSD). Laurie is excited for the opportunity to work as a team at ISHC in serving the residents of northern NH and VT!
Bryn Jay, LICSW
Bryn has been working in the North County for the last nine years at the Mental Health Center in Groveton and Colebrook. . He appreciates and is excited to be a part of the Behavioral Health Team here at ISHC as it works to provide integrated health care to the residents of Northern New Hampshire and Vermont. He has found a place here in the North Country and appreciates the many opportunities that only a rural community can offer. He looks forward to continuing to serve the Behavioral Health needs of the community.
Gregory Lamm, LICSW
Greg Lamm is a highly experienced and skilled licensed independent clinical social worker. He comes to the health center from Buffalo, WY where he was the county manager of mental health services. Greg had been looking for an opportunity to move back to his family home in North Stratford and is pleased and excited to be here at ISHC and in the North Country.
Outreach Services provide assistance with medications, applications for assistance, and community education.
Our Care Management Team supports patients with follow up scheduling, prescription refills, and help with diagnostic testing.
