Our Care Management Department works closely with the providers and care teams to focus on patient needs. We complete insurance prior authorizations for medications and diagnostic testing such as CT Scans, MRI's, and nuclear studies. Our Care Management team provides education, information, direction and support to our patients. They coordinate community services such as Meals on Wheels and can arrange transportation to appointments if needed. Follow-up phone calls are completed after discharge from the hospital to ensure that patients understand their discharge instructions and have all the necessary services required for their recovery (i.e. home health, nursing home placement etc.) One of our main goals is to increase Outreach services to our patient population. This involves healthcare screenings, patient education programs and assistance with applying for Medicaid and other sources of support for healthcare services.
