HOURS:

Monday, Thursday, and Friday 
1 pm - 5 pm

Located on the Lower Level of the Health Center.

  • Provides confidential services to women and men for physicals, pregnancy testing, STD testing and counseling, and birth control dispensing on site.

 

  • Our Family Planning services offer birth control pills, depo-provera injections, condoms, patches and emergency contraception.

 

  • No one will be denied services based on ability to pay. Insurance is also accepted and sliding fee scale would be applied after insurance payment.



Family Planning Services are provided to individuals of childbearing age in New Hampshire who request services. Special emphasis is placed on serving adolescents and individuals in low-income families, defined as less than 250% of the Federal Poverty Guidelines.



Title X (ten) funding from the federal program requires clients to complete a household assessment for Gross Family Income.

 

  • This must be completed on the first visit, annually, and with changes in financial status
  • Proof of income is required
  • Gross Family Income is defined as a social unit composed of one or more persons living together as a house hold
  • Minors that are "confidential"clients are able to claim only their income to determine eligibility for sliding fee scales.

  • New clients will have 7 days to return paperwork and proof of income
  • If the paperwork is not returned, then client will be considered a full self-pay client without any discount
  • Services will not be withheld for any client that is unable to pay
  • Unwillingness to complete paperwork is NOT considered an inability to pay

 


 

Confidential Patients:

 

Although HIPAA defines privacy of medical information, Family Planning Services allows an option to extend "confidential" services that allows the patient the opportunity to not receive bills at home, phone calls at home, or to have a shared insurance policy billed for services. This option is most often used by minor clients but is available to any client requesting it.

  • All confidential patients must make payment arrangements with the family planning program
  • Charges will be based on the house hold assessment
  • Patients that are 18 years or older may claim themselves as one in their household only if they are not included in another house hold assessment on file. There are two options for this situation:
  • Ask to be removed from the other house hold assessment and claim self as one (must have permission from primary account holder) or;
  • Accept the fee scale on the other household assessment for application to Family Planning charges
