Indian Stream Health Center seeks nominations for annual community service awards

“On a daily basis, individuals and non-profit organizations work tirelessly to address the needs of area residents,” said Jordan Phinney, Patient and Financial Services Director at ISHC.  “Our annual awards seek to recognize and thank those who have helped to make our community a healthier place to live in, work in and raise a family.”

 
Nominations can be made in three categories:

  · Community Health Advocate Award

  · Community Service Award

  · Tricia McGuire Youth Health Advocate Award

The nomination deadline is Friday, September 8th

Indian Stream Health Center releases community newsletter, establishes Twitter

As part of its ongoing effort to share news and information about its practice, Indian Stream Health Center is adding a community newsletter and a twitter handle.
 
“In addition to our daily core mission of patient care, we feel it is also important to provide avenues through which our patients and the community can access news and information about our practice,” said Jonathan Brown, CEO, Indian Stream Health Center (ISHC).  “These two new elements, along with our Facebook page, offer ways to learn about practice happenings.”
 
Mr. Brown went on to say that the newsletter-BeWell-would be released about six times a year and that patients and others could pick up a copy at ISHC’s locations in Colebrook and Canaan, VT as well as at the North Country Chamber of Commerce.  The newsletter will also be on ISHC’s website and be sent out to via email.  Anyone wishing to receive the newsletter going forward can email ISHC at yourhealth@indianstream.org.
 
ISHC’s twitter handle is @yourhealthISHC and Mr. Brown noted that along with Facebook, the new handle would provide “real time” news and information, details on community events and even information about office closures in the event of inclement weather.

The North Country Chamber of Commerce and Indian Stream Health Center (ISHC) are pleased to announce that the Health Center will be hosting a North Country Chamber of Commerce Business After Hours event at its Colebrook location, 141 Corliss Lane, on Thursday, September 21 at 5:00 p.m.

“We are looking forward to welcoming the business community and other guests to our event,” said Jonathan Brown, CEO of ISHC. “This will be a terrific opportunity for chamber members to learn more about our organization and how we are working to support a healthy North Country.”

Indian Stream Health Center holds special event in recognition of National Health Center Week
Like community health centers across the country, Indian Stream Health Center is pleased to serve as a critical provider of health care and services to those in need. “Community health centers are a vital component of the nation’s safety net,” said Jonathan Brown
, CEO, Indian Stream Health Center (ISHC). “We’re proud to serve in that role and be part of the foundation of a healthy community.”

During the week of August 13-19, ISHC will join with health centers across the United States in celebrating their daily mission of providing affordable, quality primary care and behavioral health services to children, teens, adults and seniors. Mr. Brown noted that ISHC’s locations in Colebrook, NH and Canaan, VT would be recognizing and thanking patients during the week in special ways. “Our patients have entrusted us with their health care and we take that responsibility very seriously,” he said.

26th Annual North Country Moose Festival takes place August 25-26

An annual celebration worthy of New England’s largest land mammal kicks off on Friday, August 25 with the opening of the 2017 Moose Festival. Indian Stream Health Center honors Selena Lambert
In recognition of exceptional patient service, Indian Stream Health Center (ISHC) is proud to recognize Selena Lambert with the annual Nancy Rouleau Exceptional Service Award. “Selena is a wonderful example of our caring and responsive culture,” said Jonathan
Brown, CEO, ISHC. “She’s a vital member of our behavioral health team and focuses daily on providing outstanding, compassionate care for those we serve.” Named in honor of retired employee Nancy Rouleau, the annual award is given to Indian Stream Health Center staff who go above and beyond in support of the Center’s mission. Mr. Brown noted that the award was first given out in 2009.
Ms. Lambert is a Case Worker and has been with Indian Stream Health Center since 2015. “Selena provides a vital link between our services and the community,” noted Laurie Connors, LICSW, Director of Behavioral Health Services at ISHC. “ISHC works to treat the whole patient by integrating behavioral health services with clinical care,” said Ms. Connors. “Selena tirelessly embodies this effort,

Indian Stream Health Center offers extended office hours and same day appointments
As part of its continuing mission to provide health care services to North Country residents and visitors, Indian Stream Health Center (ISHC) has unveiled a two-phase approach to expand access. Effective immediately, the Center has expanded hours in both its
Colebrook, NH and Canaan, VT locations and is offering same day appointments. “The community need is certainly there,” said
Jonathan Brown, CEO of Indian Stream Health Center. “The need for health care services is not confined to the ‘9-5’ business day so we felt it was important to extend our hours to support busy individuals and families. We’re also pleased to provide the option of
same day appointments - even for those new to our practices.”

The new office hours are as follows:

Monday - Friday: 7:00 a.m. – 7:00 p.m.
Saturday: 8:00 a.m. – 12:00p.m.

CANAAN, VT LOCATION:

additions to its clinical team. They are: Mackenzie Dufour-Physician Assistant (PA), Linne Humbargar–Physician Assistant (PA) and
Yoomi Lee, Advanced Practice Registered Nurse (APRN). “We’re excited to bring Mackenzie, Linne and Yoomi on board,” said Peter Moran, MD, Medical Director at ISHC. “Each brings solid experience and skills and will add important depth to our team.” Dr. Moran

Indian Stream Health Center Names Chief Health Officer

As part of an ongoing effort to ensure effective and coordinated health care for the communities it serves, Indian Stream Health Center (ISHC) has named David J. Amin, MD, MBA as its Chief Health Officer. “We’re excited to have Dr. Amin aboard to oversee and direct our patient-centered efforts,” said Jonathan Brown, CEO, Indian Stream Health Center. “This is a newly created position and we strongly feel this effort will raise the health and wellness bar for those we serve.” Dr. Amin joins ISHC from the Greenville Health System (GHS) in Greenville, SC-part of the University of South Carolina’s School of Medicine. At GHS, Dr. Amin served as Vice Chair of Medical Staff Affairs and Physicians, Department of Emergency Medicine. In this role, he managed and led quality control, peer review and operational process improvements across a 7-hospital system.
