Job Description: Primary Care Physician

SUMMARY: The Primary Care physician provides high quality medical services to patients of the Indian Stream Health Center (ISHC), a Federally Qualified Health Center which receives federal funding to assure health care to all populations without regard to ability to pay. ISHC promotes the Patient Centered Medical Home Model which provides primary care for patients of all ages that is “accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective” The primary care physician is responsible for leading a team of individuals who collectively take responsibility for the ongoing care of patients. The team may include a mid-level provider, nurses and administrative support staff. In addition, resources within the health center assure comprehensive care and includes proactive care management, referrals and insurance verifications, as well as outreach and behavioral health.

 

RESPONSIBILITIES:

     
  • Provide quality care to patients.
  • Lead a team of professionals that may include a mid-level practitioner, clinical and administrative support staff.
  • Adhere to the highest standards of medical practice, ethics, and professionalism at all times.
  • Assist the Medical Director and Quality Assurance/Clinical Operations Director in establishing medical policies, quality improvement procedures, evaluating current practices, policies and procedures.
  • Provide consultation to mid-level providers.
  • Participate in quality review activities.
  • Become an active medical staff member at Upper Connecticut Valley Hospital (UCVH), a neighboring Critical Access Hospital.
  • Participate in the care of in-patients as required at UCVH.
  • Maintain an affiliation with professional groups, medical associations and medical organizations as per policies established. Other duties as required by the funding agencies to carry out contractual agreements and/or as established by ISHC Management and Board of Directors.
     
    • Must become familiar with the Federal and State rules, regulations, and practices in a Federally Qualified Health Center
       
    • Become familiar with program requirements of an FQHC and assist in carrying out those programs.
       
  • Promote the ISHC mission.
  • Ability to relate to the public regardless of ethnic, religious or economic status.
  • Other duties as assigned.


 

EMPLOYMENT QUALIFICATIONS

  • Graduation from an accredited school of medicine
  • Completion of a residency program in either Family Practice or Internal Medicine
  • Must have a valid New Hampshire Practice License
  • Professional clinical experience in specific area of practice of primary care practice, either Family Practice or Internal medicine
  • Board certification or eligibility for said certification in Family Practice or
    Internal Medicine
  • Experience or additional training in geriatric care and the management of chronic disease
    • Sitting in a normal seated position for extended periods of time
    • Reaching by extending hand(s) or arm(s) in any direction
    • Finger dexterity required to manipulate objects with fingers rather than with whole
      hand(s) or arm(s), for example, using a keyboard
    • Communication skills using the spoken word
    • Ability to see within normal parameters
    • Ability to hear within normal range
    • Ability to move about


 

CLASSIFICATION: Full time, exempt position
