Family Practice Medicine: Comprehensive primary health care, regardless of the age or sex of the patient, with special emphasis on the family unit. Adult Medicine: Focusing on the prevention and treatment of adult diseases. Pediatric Primary Care: Comprehensive medical treatment for children from birth through adolescence, including preventive care, treatments of acute illnesses, as well as ongoing treatment of chronic conditions. Women's Health: Complete information and instruction on preventive health care strategies for women, as well as exams, screenings, and resources for uninsured patients. Behavioral Health: Focusing on the connection between behaviors and the health and well-being of the body, mind and spirit. This would include how behaviors and issues impact physical or mental health. Individual, Family, and Group Therapy: Support and conflict resolution. Help with mental health concerns, depression, anxiety, domestic violence, etc. Retail Pharmacy: ISHC pharmacy is a full service pharmacy, offering a large selection of over-the-counter items in addition to retail and 340B prescription services. One does not have to be a patient of ISHC to use the pharmacy. Low Cost Pharmacy Program: Help for eligible individuals with medication costs. Care Management: Our Care Management Team assists patients with coordination of their care through follow-up, assistance with referrals and plan of care development. Outreach Services: Outreach Services provide assistance with medication assistance programs, insurance applications and enrollment (including Medicare, Medicaid, and Qualified Health Plans), advance directives, and other community programs.
